Chicago Family Health Center, a Federally-Qualified Health Center is a 501 (c)(3) community health center operating five locations in South Chicago, East Side, Roseland, Pullman and Chicago Lawn. The center has 29 Family Practice, Internal Medicine, Pediatrics, OB/GYN and Cardiology physicians, five dentists, four behavioral health providers and more than 225 staff members providing support services and wrap-around care. The center manages more than $7 million in grants and contract funding and provides primary health care to more than 24,000 unique individuals. The center also provides care in 24 homeless shelters throughout the Southland. With a budget of approximately $20 million, Chicago Family Health Center is poised for continued growth through additional services and delivery sites. - Chief Operations Officer
Five years experience at the executive level, preferably in a community health center or safety-net hospital, Bachelor’s Degree, and proven track record in cost-effective operations leadership. Financial and business acumen, familiarity with health care accreditation program, health care industry and Federally-Qualified Health Center regulations are a plus. - Nurse Practitioner
MS Degree from accredited nursing school, current and valid Illinois license as an APN or eligible for certification required. - Licensed Clinical Social Worker
MS Degree in Social Work, with a current and valid Illinois License and minimum of 2 years experience in a public health environment. - RN
Bachelor’s Degree or Associates Degree in Nursing with current and valid Illinois RN license, 2 year experience in women’s health program or community health setting preferred. - Electronic Health Records Specialist
BS Degree in Computer Science or equivalent work experience. 2 years minimum experience in EHR system implementation, EPM system and end user support.
