Chicago Family Health Center is a community-based, non-profit health center that provides comprehensive, accessible primary health care to the residents of South Chicago, Roseland, Pullman and surrounding communities. The Chicago Family Health Center provides health care to all people. Services are provided regardless of their race, religion, language, gender, age, income or their ability to pay.

Cash Gifts
Cash gifts are the most popular because they are simple to make, immediately effective, tax-deductible, and easily earmarked for the current needs at CFHC. To make a donation, please send checks payable to Chicago Family Health Center to the External Affairs Department, Chicago Family Health Center, 9119 S. Exchange Ave., Chicago, IL 60617.

Matching Gift Program
Take advantage of your company’s matching gift program, if available, and double or triple the value of your gift to the CFHC. Companies may match securities as well as cash. Just ask your company’s matching gift officer (usually in the human resources or community relations department) for a matching gift form. Send the matching gift form to the CFHC with your personal contribution, and we’ll complete the paperwork.

Workplace Campaigns
Many corporations and public sector employers in Illinois have organized workplace campaigns. Please designate CFHC in your workplace campaign, which will allow you to make monthly donations.

Memorial and Tribute Gifts
Why give a standard, gift-wrapped present when you can memorialize or honor someone and benefit the community at the same time? CFHC will notify the honoree(s) of your gift by sending a wonderful, personalized card with your personal message and provide you with a tax-deductible letter for your records. Memorial and tribute gifts will be noted in our newsletter, annual report and on our Web site.

Corporate and Foundation Giving
Corporate Grants
Corporate contributions and foundation grants are appreciated at any level, but corporations that give $1,000 and above receive recognition, tours, and invitations to special receptions. Special opportunities exist to make grants to meet CFHC's specific program needs.

Foundation Grants
CFHC actively seeks programmatic, unrestricted and capital grants from area community and private foundations to provide necessary funding for its programs and facility. If you make your gifts through a foundation, please let us know if CFHC would be eligible for consideration.

Pledges
Pledges provide prospective donors with additional flexibility in planning a gift to the CFHC. Donors may make their commitments payable over a mutually agreed period of time.

Planned Giving
The CFHC’s Planned Giving program recognizes friends and families who have included the Chicago Family Health Center in their estate plans. If you notify us that you have made the CFHC a beneficiary in a life insurance policy, charitable trust or will, we will include you in all materials and publications that acknowledge our Planned Giving Program members. No minimum dollar amount is necessary to qualify. Please contact our Director of External Affairs if you have designated the CFHC as a beneficiary under a life insurance policy, trust or will, so we can thank you for your promised gift.
