Homeless Primary Care

Working collaboratively with Heartland Health Outreach, CFHC nurse practitioners and staff provide primary care and case management services at 25 homeless shelters on the south side of Chicago. When follow-up care is required, we refer clients to one of our three clinics or to other community programs. We serve more than 1,300 homeless clients in more than 5,400 medical, dental and case management visits each year.
