CHILDREN’S HEALTH
Chicago Family Health Center (CFHC) offers excellent primary care for children from the time they are infants, into their years as toddlers, children, adolescents and young adults. We have a team of dedicated pediatricians and primary care providers that can meet the healthcare needs of your children through the various stages of their growth and development.
WOMEN’S HEALTH
Chicago Family Health Center (CFHC) provides health care services for every stage of a woman’s life — from puberty and pregnancy through menopause and beyond. Our Women’s Health services include a full range of medical services for women of all ages and provide comprehensive care for routine, urgent and chronic medical needs, as well as sexual health, reproductive and gynecological services, immunizations, and referrals.
BEHAVIORAL HEALTH

MEN’S HEALTH
At Chicago Family Health Center (CFHC) we encourage all men to have a Primary Care Provider and to come in for regular check-ups. We provide confidential, professional treatment for a variety of men’s health needs, and offer a full array of health and wellness programs that address the three pillars of good health including: Primary, Behavioral and Dental Care.
CHRONIC ILLNESS
Chronic illnesses or long-term health conditions include high blood pressure, asthma or diabetes. We offer healthcare programs that not only address the symptoms that these conditions cause, but also help you manage your treatment and overall healthcare plan so you can maintain a better quality of life.
MEDICATION/PRESCRIPTIONS
Chicago Family Health Center (CFHC) provides phone or in person-assistance at our South Chicago site with getting insurance authorization for medications, or if the pharmacy needs additional verification. If your insurance does not cover your prescription, the dispensary technicians will help you apply for patient assistance if you qualify for discounted medications.
