Making an appointment is easy. To make an appointment at any of our locations, call 773-768-5000. Please tell the call center operator what services you need, which CFHC site is closest to where you live and have your insurance information handy.

Patient Portal

As a Chicago Family Health Center (CFHC) patient, you can access your medical records and communicate with your care team from the comfort of your computer or smart phone. The Patient Portal is a secure website that lets you request non-urgent appointments, request prescription refills, view lab results, ask billing questions or receive referrals.

If you are a new patient, you can ask the reception staff to enroll you in the portal when you come in for your first appointment. Current CFHC patients can self-enroll into the Patient Portal by clicking here, completing the fields marked with an asterisk (*) and hitting “Submit.”

What to bring for your appointment

On the day of your appointment please arrive on time and bring the following information, which is required to complete your patient registration:

Photo identification

One form photo Identification is required such as:

  •     Driver’s license
  •     State identification
  •     Matricula card
  •     Employment or school ID
  •     Public aid ID
  •     Social Security card
  •     Alien registration card/”green card”
  •     Valid passport
  •     Veteran’s ID

Your current address and phone number

Medical card or insurance card (if you have one)

If you don’t have a medical card or insurance, please bring income information, such as:

  • Two paycheck stubs
  • W-2 form
  • Medicaid card
  • Food stamp card
  • Unemployment notice
