Since its founding in 1977, Chicago Family Health Center (CFHC) has become the primary care medical home for thousands of Chicagoans, including some of the city’s most vulnerable populations. As an award winning, nationally-recognized leading voice for community health centers, CFHC has played a vital role in providing access to affordable, comprehensive healthcare services to underserved communities throughout Chicago.

Chicago Family Health Center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
Born out of the underserved medical needs of Chicago’s south and southeast side, Chicago Family Health Center (CFHC) has grown to become a leading medical network, here to serve anyone in Chicago who desires high quality healthcare.
During the 1970s, Chicago’s south and southeast sides were struck with a major economic downturn, where more than 43,000 area residents found themselves without jobs and in need of social, financial and medical support. The Claretian Missionaries recognized the dire need of the affected neighborhoods and brought together community residents and other stakeholders to create a health safety net that would provide quality healthcare for area residents. In 1977 CFHC, then known as Claretian Medical Center — opened its South Chicago clinic at 91st Street and Commercial Avenue. That year, it provided care to 400 community residents.
Our health network currently includes locations in South Chicago, Pullman, East Side, Roseland and Chicago Lawn. Today, CFHC provides accessible, affordable, high quality healthcare for more than 30,000 underserved, uninsured and underinsured patients throughout Chicago.

Our capital investment of building new state-of-the-art facilities in underserved areas help serve as an economic catalyst by providing continued employment to area residents, the purchase of goods and services from local businesses, as well as other economic stimuli.

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
