CFHC at CVCA is a school-based health center offers an integrated model of care including primary and behavioral health services right on the CVCA campus. The clinic is open to the student body, their parents/guardians as well as community residents. In addition, support services for the whole family including care coordination and the Women, Infants and Children (WIC) program can be accessed at our South Chicago site, 9119 S. Exchange Ave.

If you have a medical problem that needs attention after clinic hours, we have staff on call 24 hours a day at 773-768-5000.
Directions to the clinic: From 87th Street you will turn on to Chappell St. to access the school parking lot where the clinic is located.
