About Chicago Family Health Center:
Since its founding, Chicago Family Health Center (CFHC) has served as the primary care medical home for thousands of Chicagoans, including some of the city’s most vulnerable populations.
CFHC plays not only a vital role in our patient’s health, but also as an essential resource that strengthens the communities we serve. Our mission goes beyond expanding access to quality health care services. We are committed to investing in the lives of the residents of Chicago.
