Chicago Family Health Center offers a wide range of health programs and services to meet your and your family’s healthcare needs. Whether you need an annual health exam, vaccines, chronic disease management or your children need a back-to-school checkup we are here for you. Our clinics also offer behavioral health, support services and dental care. In-person and telehealth appointments are also available.

New Patients

The easiest way to make an appointment is to give us a call at 773-768-5000. Our friendly Patient Service Representatives are on hand to guide you through your healthcare journey.

Current Patients

If you’re an existing patient, you may view your health information via our online Patient Portal. Additionally, you may call 773-768-5000 anytime you need to make an appointment. Simply let us know what services you need, your clinic location and the name of your Provider. If your insurance has changed, we will ask for the updated information as well.

What to Bring To Each Appointment

On the day of your appointment, please arrive early to complete registration. We will ask you to verify your current address and phone number.

  • Driver’s License
  • State ID
  • Matricula Card
  • Employment or School ID
  • Public Aid ID
  • Social Security Card
  • Alien Registration Card or Green Card
  • Passport
  • Veteran ID

Insurance & Payment

We accept most major insurance plans (including Medicaid) and offer a sliding scale for those without insurance.

If you don’t have a medical card or insurance, click here to see if you qualify for a plan or for our Sliding Scale discount program.

If you don’t have insurance, please bring income information. Examples include:

  • Two paycheck stubs
  • W-2 Form
  • Medicaid Card
  • Food Stamp Card
  • Unemployment Notice

Translation Services

Our patient community spans a wide range of cultures and backgrounds. To meet everyone’s needs, we provide translators to make patients feel welcomed and understood at their visit. These professional interpreters speak over 300 languages and specialise in healthcare translation.

After-Hours Care

After-hours care is available to you and your family. By calling 773-768-5000 when CFHC is closed, you will be connected with our answering service for assistance. If there is an urgent medical need you will be put in touch with our medical staff. Bilingual staff is also available to you.
