Our support services provide a holistic approach to patient care. If a patient comes in for a regular doctor’s visit but needs additional services, our Care Management team is here to assist. The team is made up of dedicated nurses and Care Coordinators who work side by side with the medical provider and other clinical staff to connect patients with needed resources such as:
Care Coordinators guide you through the different levels of services you request or require, ensuring that your healthcare and social needs are addressed. Resources include but are not limited to assistance with scheduling appointments, transportation, housing, and food.
Care Managers are licensed personnel equipped to provide disease/medication management. They also provide education for complex cases assisting patients to move from high-risk behaviors and conditions to lower-risk self-management behaviors. Our Patient Resource Counselors (PRC) offer a variety of services and financial assistance at many of our health center sites. They can help you enroll in an insurance plan that is right for you and your family. If you do not qualify for health insurance, they can sign you up for our sliding scale program which can offer discounted costs for healthcare services depending on your income. Nutritionists work with physicians to conduct individual and group classes on proper nutrition, improving your diet and bettering your eating habits. Good nutrition provides energy, vitamins and minerals and can help you battle the symptoms of many chronic conditions, such as high cholesterol, diabetes and high blood pressure.
When adults develop healthy eating habits, children and families will follow suit.Teaching kids to eat right and stay active will keep them from reaching dangerous weight levels and instill in them lifelong awareness of the importance of good nutrition.
The nutritionist is available for appointments through a referral from their CFHC provider. No co-pay is necessary.
