New to CFHC? Enroll in our Patient Portal
CFHC has a new Patient Portal system. If you want to take advantage of the Patient Portal you must enroll, or re-enroll into the new system. If you were enrolled in the old system you must enroll into the new system, however you can still log into the old portal with your sing-in credentials to access anything in your medical record prior to January 15, 2019. The CFHC Patient Portal is still the best way to access your health record, receive referrals and prescriptions as well as request appointments and contact your Provider or health care team.

As a Chicago Family Health Center patient, you can access important medical records and schedule appointments from comfort of your home, work computer, or anywhere you have Internet. This secure website lets you:
