MyChart
We’re launching MyChart, our new secure patient portal powered by Epic. MyChart gives patients one place to manage their care online, including appointments, test results, visit information, billing, and secure messages with the care team.
How to Access to MyChart?
You can get access to MyChart after you make an appointment.
To sign up, you will need an activation code from Chicago Family Health Center. This code will be provided after your appointment is set up. Once you receive your code, you can complete your MyChart signup and begin using your account.
With MyChart, patients can:
– Schedule and manage appointments
– View test results
– Review medications, visit summaries, and other health information
– Send secure messages to their care team
– Request prescription refills
– View and pay medical bills
