If you are experiencing a crisis, we provide stabilization support and safety planning to help you navigate challenging situations. Additionally, we connect patients with specialized mental health and substance use disorder treatment referrals when more intensive care is needed.

We believe in an integrated, person-centered approach that allows the patients to drive their own recovery with the support of the treatment team: the medical provider, the behavioral health provider, and the care navigators.
CFHC offers Suboxone, Naltrexone and Narcan. To receive Suboxone prescriptions, you must have CFHC listed as your Primary Care Provider on your insurance plan, if applicable. Our staff can help you switch your medical care if needed. Patients can also receive referrals to specialty partner agencies for other Substance Use Disorder needs.
