We offer home monitoring for patients with chronic hypertension. Patients receive FREE blood pressure monitors to encourage them to check their blood pressure at home on a regular basis. 

A CFHC clinical team monitors their readings remotely and provides proper instructions, information, and resources. Patients can also connect with their care team from home. FREE devices preloaded with free internet are also available to anyone without access to the internet.

We offer diabetic retinopathy screenings at our South Chicago, Pullman, Roseland, East Side and Chicago Lawn clinics.

  • Patients 21 years and older diagnosed with Type II Diabetes automatically receive a lab for a screening, or they can be screened during their visit with their Provider.
  • Patients may also receive a phone call alerting them that they are due for a diabetic retinopathy screening. 

The procedure can be scheduled at that time.

What is Diabetic Retinopathy?

An eye disease that can affect people with type 1 or type 2 diabetes and can cause serious complications, including vision loss or blindness.

What is this test?

A comprehensive eye exam that is performed by an eye care professional to check for signs of diabetic retinopathy.

How many times do I need to be tested?

It’s important that your eyes are tested every year because there are no symptoms until the disease is in its later stages. It can cause blindness if left untreated.

How long does this exam take?

The exam typically takes 5-10 minutes. It may take a little longer if the technician has difficulty capturing good images of your eyes, usually due to patients having smaller pupils.

Will I need dilation?

About 3/4 of patients can complete this exam without dilation. A small percentage of patients require a mild dilation drop to open the pupil slightly. You can bring someone with you to drive you home if that makes you comfortable, but most patients are okay with the use of sunglasses, or a sun shield which we provide. 

I get my vision checked every year, and I just got my new glasses. I don’t believe that I need this exam.

A vision test is not the same thing as a medical exam for diabetic retinopathy. Your provider wants to make sure your diabetes isn’t threatening your vision with diabetic retinopathy.
